(1) Fill out Questionaire Form, available at the bottom of this page (See instructions for downloading Questionaire Form Below).
(2) Fill out Registration Form .
(3) E-mail completed Registration Form and Questionaire to newcastlejaycees@gmail.com.

Registration Form and Payment must be received by Tuesday, April 6, 2010.  When Registration Form and Payment is received, organizers will e-mail you confirming your participation.
