Directions:
(1) Fill out the Questionnaire, which may be downloaded below. Answer truthfully and keep it clean. The answers to the Questionnaire will be incorporated into our ice breaker games.
(2) E-mail completed Registration Form and Questionnaire to newcastlejaycees@gmail.com.
In order to complete the questionaire, please do the following:
(1) On the small "
" button below, Right click on the icon and select "Save Target As...". The file is in Microsoft Word format. (if you do not have MS Word, click here)
(2) Open the file and complete the requested information.
(3) Save the file again.
(4) To submit the registration file, send an E-mail to the event coordinators at newcastlejaycees@gmail.com and attach the file to the E-mail.
