Date: Thursday, August 20th, 2009
Time: 7:00pm-9:30pm (Check-in begins at 6:30pm)
Where: Kooma (400 Justison Street, Riverfront, Wilmington)
Cost: $20 at the Door; $15 in Advance; $10 for Jaycees
Who: Ages 22-40
Directions:
(1) Fill out Registration Form, available at the bottom of this page (See instructions for downloading Registration Form Below).
(2) Check appropriate Payment Method. Use Paypal or follow payment method instructions on Registration Form.
(3) Fill out Questionnaire .
(4) E-mail completed Registration Form and Questionnaire to newcastlejaycees@gmail.com.
Registration Form and Payment must be received by Wednesday, August 19, 2009. When Registration Form and Payment is received, organizers will e-mail you confirming your participation.
Note: All matches will be sent to your e-mail address. Please check appropriate lines to allow organizers to release info to matches.
In order to register, please do the following:
(1) On the small "
" button below, Right click on the icon below, and select "Save Target As...". The file is in Microsoft Word format. (if you do not have MS Word, click here)
(2) Open the file and complete the requested information.
(3) Save the file again.
(4) To submit the registration file, send an E-mail to the event coordinators at newcastlejaycees@gmail.com and attach the file to the E-mail.
